3. Communication with patients and their families(considering age, gender, sexuality, ethnic cultural and socio-economic background)

a) Demonstrate consideration of patient’s age, gender, sexuality, ethnic cultural and socio-economic background in all communication


4. Communication with members of the healthcare team

a) Communicate with other members of the healthcare team in a friendly and professional manner

b) Answer pages/e-mails/cell phone calls promptly and in a professional manner

c) Maintain clear, accurate and appropriate records (written and/or electronic) of all clinical encounters and plans


5. Communication with referring and/or family physician (consult notes, discharge summaries, phone advice)

a) Convey relevant information, including consult notes, discharge summaries, and telephone communication to colleagues and other health professionals (such as referring/family physicians), in an accurate and timely fashion


6. Ensure that consult notes are dictated promptly and accurately

a) Ensures that all consult letters and notes are dictated promptly and accurately, including the following components:

      • Subjective history
      • Objective findings, including physical examination and laboratory investigation
      • Summary/impression
      • Care plan
